Job summary

Angels Care Hospice is seeking a Registered Nurse Case Manager to coordinate patient home care, perform admissions, and develop individualized plans of care. You will collaborate with other medical disciplines to deliver professional nursing services in patients’ homes.

The role requires an active RN license, a valid driver’s license, and at least one year of RN experience (two preferred). CPR and hepatitis consent/declination are required, with reliable transportation.

Qualifications

• Graduate of an accredited nursing program and licensed as a Registered Nurse in the state.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• Minimum one year RN experience; two preferred.
• CPR certification and Hepatitis consent/declination.
• Reliable transportation with auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care and interact with other medical disciplines.
• Conduct admission visit to patient in the home.
• Determine patient eligibility for home care services and develop the plan of care.
• Provide professional nursing care and liaison with care team to ensure quality outcomes.

About the position:

The Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home, determine the patient’s eligibility for home care services, & develop the plan of care to be followed.

Qualifications:

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.
