Job summary

Angels Care Hospice is seeking a Hospice Registered Nurse (RN) Case Manager to coordinate all aspects of home care for patients in Omaha, NE. You will conduct admission visits, assess eligibility for home care services, and develop the care plan while collaborating with other medical disciplines.

The role offers a $5,000 sign-on bonus for full-time staff, mileage and on-call pay, and comprehensive benefits along with extensive training.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care as RN Case Manager.
• Interact with medical disciplines as needed and provide professional care to the patient.
• Perform admission visit in the home, determine eligibility for home care services, and develop the plan of care.

About the position:

The Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care. You will interact with other medical disciplines as needed & provide professional care to the patient.
