Job summary

Athena Health Care Systems seeks an experienced Hospice RN Case Manager to plan, organize and direct hospice care for patients and families throughout Barnstable and Plymouth Counties. This on-the-road role offers up to $50 an hour, plus weekly pay, with mileage reimbursement and benefits.

Candidates must have an active MA RN license, 1–2 years acute care and home care experience, CPR certification, and a valid driver's license with reliable vehicle.

Qualifications

• Graduate of an accredited school of nursing.
• Must possess an active and unencumbered RN license in Massachusetts.
• 1-2 years of recent acute care experience in an institutional setting, and 1-2 years of recent experience in home care preferred.
• CPR certified.

Responsibilities

• Supervise hospice aides.
• Complete comprehensive assessments of patient and family to determine hospice needs.
• Provide professional nursing care by utilizing all elements of the nursing process.
• Develop and implement individualized care plans.
• Collaborate with interdisciplinary team members to ensure comprehensive care.
• Provide health care instructions to patients and families.

Per diem Hospice RN Case Managers with Hospice Services of Massachusetts

We are seeking an experienced and compassionate RN Case Manager to join our hospice team. As a Hospice RN Case Manager, you will plan, organize, and direct hospice care for our patients and their families.

Up to $50 an hour, depending on experience, with weekly pay.

This is an on-the-road position that requires the candidate to have a driver's license and an insured and reliable vehicle. Candidate must be willing to travel to portions of Barnstable and Plymouth Counties.
