Job summary

Residential Home Health and Residential Hospice is seeking a Hospice Registered Nurse to provide end-of-life care in patients' homes around Milford, PA. The role offers autonomy, meaningful patient relationships, and opportunities for growth within a nationally recognized home health and hospice leader.

The position includes a $10,000 first-year success bonus and requires an active RN license, two years of RN experience (hospice preferred), and a valid driver's license with travel to patient

Qualifications

• Active RN license in the state of work.
• Two years of RN experience; hospice experience preferred.
• Valid driver’s license and ability to drive ~20% of the day.

Responsibilities

• Deliver compassionate, patient-centered hospice care in patients’ homes.
• Perform comprehensive assessments, symptom and pain management, and end-of-life planning.
• Educate and support patients and families through the journey.
• Maintain thorough EMR documentation and compliance.
• Work independently within an interdisciplinary team.
• Travel to patient homes within the assigned territory.

What You’ll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life

• Perform comprehensive patient assessments, symptom and pain management, and individualized end-of-life care planning

• Educate and support patients and families through the end-of-life journey, ensuring they feel informed, prepared, and deeply cared for

• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

• Work independently, but never alone

• Travel to patient homes within assigned territory

RN Qualifications

• Active Registered Nurse, RN license in the state you work
• Two years of RN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
