Job summary

Graham Healthcare Group is hiring a Registered Nurse (RN) for a Hospice role delivering compassionate end-of-life care in patients' homes in the Middleburg, PA area. Eligible for a total bonus potential of $15,000, including a $10,000 first-year success bonus and a $5,000 sign-on bonus after 30 days.

You’ll work with an interdisciplinary team, travel to patient homes, and impact quality of life while building your career in a nationally recognized home health leader.

Qualifications

• Current RN license in the state you work in.
• Hospice experience preferred for patient-focused end-of-life care.
• Must have a valid driver’s license and be able to travel ~20% of time.

Responsibilities

• Deliver patient-centered hospice care in patients' homes with a compassionate, interdisciplinary team.
• Conduct comprehensive assessments, symptom and pain management, and care planning.
• Educate and support patients and families through the end-of-life journey.
• Document thoroughly in EMR and maintain compliance.
• Work independently while staying connected to the team and schedule.
• Travel to patient homes within assigned territory.

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Perform comprehensive patient assessments, symptom and pain management, and individualized end-of-life care planning
• Educate and support patients and families through the end-of-life journey, ensuring they feel informed, prepared, and deeply cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

RN Qualifications

• Active Registered Nurse, RN license in the state you work
• Two years of RN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
