Job summary

Residential Home Health and Residential Hospice in Danville, PA is hiring a Hospice Registered Nurse to deliver end-of-life care in patients' homes, collaborating with an interdisciplinary team to improve comfort and quality of life.

You will perform assessments, manage symptoms, educate families, and document care in the EMR. This field-based role requires travel approx. 20% and offers a $10,000 first-year milestone bonus.

Qualifications

• Active RN license in the state you work.
• Two years RN experience with hospice preferred.
• Valid driver’s license and ability to drive ~20% of work time.
• Ability to work independently with interdisciplinary team.

Responsibilities

• Deliver compassionate, patient-centered hospice care in patients' homes.
• Perform comprehensive patient assessments and pain/symptom management.

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life

• Perform comprehensive patient assessments, symptom and pain management, and individualized end-of-life care planning

• Educate and support patients and families through the end-of-life journey, ensuring they feel informed, prepared, and deeply cared for

• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

• Work independently, but never alone

• Travel to patient homes within assigned territory

RN Qualifications

• Active Registered Nurse, RN license in the state you work

• Two years of RN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred

• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
