Community Healthcare of Texas is seeking a Community Marketing Liaison to promote our hospice program to physicians, discharge planners, and other referral sources. The role acts as a liaison between hospice and the medical community to expand referrals and visibility.

The position requires a college degree or equivalent experience, 2+ years in marketing, hospice or home health experience, proficient computer skills, and strong relationship-building and communication abilities.

Qualifikationen

• College degree or equivalent experience is required.
• 2+ years of marketing experience.
• Hospice or Home Health experience preferred.
• Proficient skills using a computer.
• Must be able to build relationships and communicate clearly.

Aufgaben

• Generate referrals from physicians, discharge planners, and hospital staff to promote hospice services.
• Maintain community visibility and attend networking programs.
• Coordinate education programs in hospitals, physicians' offices, and community settings.
• Track and report monthly contacts with referral sources and submit to supervisor.
• Participate in weekly team conferences to share information and ensure quality care.
• Assist with special events, health fairs, and public relations activities.
• Assist with the admission process by meeting with patients and completing documentation.
