Qualifications

• Self-motivated with strong organizational and follow-up abilities.
• Ability to work independently while collaborating with internal teams.
• Valid Texas driver’s license and reliable transportation.
• Bilingual English/Spanish is a plus, reflecting Corpus Christi community.

Responsibilities

• Develop and maintain referral relationships with physicians, discharge planners, social workers, facilities, and community partners.
• Educate healthcare professionals on hospice services, eligibility, and benefits.
• Promote hospice awareness through community outreach and educational presentations.
• Serve as a liaison between referral sources, hospice staff, patients, and families.
• Track referrals, activity, and outreach efforts in coordination with leadership.
• Collaborate closely with the hospice leadership and intake teams.
