Job summary

Dignity Care Hospice is seeking a Hospice Account Executive to cultivate relationships with referral sources and guide patients and families through the hospice referral and admission journey. You will educate providers, hospitals, clinics, senior living communities, and families to ensure timely access to hospice services.

Ideal candidates have 1-3 years of hospice sales experience, a valid driver’s license, and previous work as a Hospice Liaison or Hospice Account Executive is preferred.

Qualifications

• 1-3 years of translatable experience in Hospice sales.
• Current and valid driver’s license with available transportation and ability to drive.
• Previous work as a Hospice Liaison or Hospice Account Executive preferred.

Responsibilities

• Building and maintaining relationships with physicians, case managers, hospital staff, senior living communities, and other referral partners.
• Coordinating hospice referrals by gathering clinical information and supporting eligibility assessments.
• Conducting community education and outreach about hospice services and benefits.
• Supporting families and patients through the referral and admission process.
• Collaborating with the IDT to share capacity and patient needs information.
• Tracking referral trends and engagement using CRM/EMR/referral platforms.
