Job summary

The Care Team Hospice is seeking a driven Community Care Liaison to grow our presence in Peoria, IL. You’ll build relationships with physicians, hospitals, and referral sources to increase awareness and utilization of hospice services.

As the face of our hospice in the community, you will educate providers, patients, and families on the value of hospice and guide them toward compassionate, high-quality care. A collaborative team supports your growth and impact.

Qualifications

• Bachelor’s degree or equivalent.
• 1-2 years healthcare outside sales experience.
• 1-2 years Hospice or Home Health sales preferred.

Responsibilities

• Build and expand relationships with physicians, healthcare partners, and key decision-makers to drive referral volume and market growth.
• Serve as a dedicated advocate for hospice by educating the community and sharing the value of comfort-focused, patient-centered care.
• Achieve and exceed census and admission targets by positioning our hospice as a preferred provider within the community.
• Engage regularly with healthcare providers and referral sources to create strong, collaborative partnerships focused on patient well-being.
• Lead hospice discussions with patients and families, clearly communicating services and guiding the consent process with professionalism and empathy.
