The Care Team Hospice is seeking a driven, relationship-focused Community Care Liaison (CCL) to grow our presence in the community. As a Community Care Liaison, you will be the face of our hospice services in the community. You’ll build and strengthen relationships with physicians, hospitals, and other referral sources to increase awareness, trust, and utilization of our services.
Your success will be measured by your ability to grow census through strong partnerships, consistent outreach, and a deep understanding of hospice care. You’ll also serve as a trusted resource—educating providers, patients, and families on the value of hospice and guiding them toward compassionate, high-quality care.

Bachelor’s degree or equivalent
Ability to perform regular travel within assigned geography and extended geography for training and meetings.
