Job summary

The Care Team Hospice in Peoria, IL is seeking a Community Care Liaison to grow its presence in the community. You will build relationships with physicians, hospitals, and other referral sources to increase awareness and utilization of hospice services.

The role emphasizes census growth through partnerships, outreach, and education for providers, patients, and families. You will be the face of our hospice, guiding them to compassionate, high-quality care, with a supportive team and growth

Qualifications

1-2 years healthcare outside sales experience.
Hospice or Home Health sales experience preferred.
Bachelor’s degree or equivalent required.

Responsibilities

Build and expand relationships with physicians, hospitals, and other referral sources to drive referral volume and market growth.
Educate providers, patients, and families on the value of hospice care.
Achieve census and admissions targets by positioning hospice as a preferred provider.
Engage regularly with healthcare providers to create strong partnerships focused on patient well-being.
Lead conversations with patients and families about services with professionalism and empathy.
