Job summary

JourneyCare Hospice is seeking a driven Hospice Liaison to grow our presence in Chicago, IL. You will own your territory, build referral partnerships, and increase census by presenting JourneyCare as the provider of choice.

This field-based role rewards accountability, visibility, and new business development in hospice, home health, or healthcare marketing. Strong relationships and communication are essential.

Qualifications

• Bachelor’s degree preferred; combination of experience and/or training may be considered.
• Minimum of one year of professional sales experience preferred.
• Experience in hospice or healthcare industry a plus.

Responsibilities

• Coordinates and facilitates referrals to Hospice care based on admission criteria.
• Assist in information gathering, education, and form completion for intake and admissions.
• Develop a consultative relationship in assigned accounts through effective customer contacts and presentations.
• Develop and implement marketing and education programs to increase awareness of the company.
