JourneyCare Hospice in Chicago, IL is seeking a driven Hospice Liaison to grow our presence in the market. This field-based role rewards accountability, visibility, and success in securing referrals and increasing census.

Qualifications

• Bachelor’s Degree preferred; combination of experience and/or training may be considered in lieu of formal education.
• Minimum of one year of professional sales experience preferred.
• Experience in the hospice and health care industry a plus.
• Demonstrates good verbal and written communication, and organization skills.
• Valid driver's license with an automobile that is insured in accordance with state requirements.

• Assist in the information gathering, education, and form completion aspects of patient intake, admissions, and transition to home

Hospice Liaison – Chicago, IL

JourneyCare Hospice is seeking a driven, high-impact Hospice Liaison to grow our presence in Chicago, IL. We’re looking for a results-oriented sales professional with experience in hospice, home health, or healthcare marketing who knows how to build relationships and generate referrals.

In this role, you will take ownership of your territory, developing strong referral partnerships, increasing census, and positioning JourneyCare Hospice as the provider of choice. This is a field-based role for someone who thrives on accountability, visibility, and winning new business.

Location: In the assigned market (Chicago, IL and surrounding).

Salary: $80,000 - $85,000 (plus performance based incentives)
