Job summary

Lifecare-Home-Health-Family-1 in Texas seeks a Patient Care Advocate to drive referral growth for hospice services. You will develop partnerships, present services to physicians and community agencies, and support patients and families with home care options.

Must have strong relationship-building, excellent communication, and the ability to travel locally. Bachelor's degree or equivalent experience and CRM/Microsoft Office proficiency are preferred.

Qualifications

• Bachelor's degree in marketing or related field or equivalent professional experience.
• Minimum of two (2) years of hospice sales or related industry experience with growth.
• Excellent relationships with internal and external teams.
• Proficiency in Microsoft Office; CRM experience preferred.
• Excellent oral communication skills; self-directed with little supervision.

Responsibilities

• Develops business partnerships by making effective sales contacts and presentations.
• Grow hospice programs and services while maintaining quality end-of-life care standards.
• Serve as liaison between referral sources, physicians, facilities, and community agencies.
• Develop quarterly and annual territory plans to achieve growth goals.
• Maintain provider relationships and communicate new products and services.
• Meet with patients and families to discuss home care services and needs.
• Track referrals, activity, and plans for follow-up with sources.
