Job summary

Infinity Hospice Care LLC is seeking a Patient Care Advocate to develop partnerships and grow referrals for our hospice programs. You will call on physicians, facilities, discharge planners, and community agencies, delivering presentations and building long-term referral relationships.

The role requires marketing savvy, two years hospice sales experience, and proficiency with Microsoft Office and CRM tools.

Qualifications

• Education: Bachelor’s degree in marketing or related field or equivalent professional experience.
• Experience: At least two years in hospice sales or related industry with growth record.
• Skills: Ability to establish and maintain professional relationships with internal and external teams.
• Must be proficient in Microsoft programs for communications, presentations, tracking, and reporting.
• Experience with CRM solutions is preferred and beneficial.

Responsibilities

• Develop overall territory plans to achieve growth goals and implement strategies.
• Make effective sales calls and presentations to targeted referral sources.
• Maintain professional knowledge of provider services and coordinate with referral partners.
• Meet with patients and families to discuss home care services and needs.
