Job summary

AccentCare is seeking a Hospice Care Consultant in Burr Ridge, IL, to lead hospice sales and build strong relationships with healthcare providers, facilities, and community partners. The role emphasizes educating referral sources and ensuring timely, quality patient care.

You will collaborate with Admissions and Marketing, develop outreach plans, and work with clinical staff to ensure smooth transitions from referral to care while upholding ethics and confidentiality.

Qualifications

• Bachelor's degree in healthcare or related field preferred.
• At least 5 years in home health, healthcare sales, or hospice sales.
• Strong communication and relationship-building skills required.

Responsibilities

• Develop and maintain trusting relationships with physicians, hospitals, skilled nursing facilities, and community organizations through visits and educational conversations.
• Drive hospice sales by nurturing relationships, identifying referral opportunities, and increasing program awareness.
• Conduct presentations and in-services to educate partners on hospice benefits and eligibility.
• Collaborate with Admissions and Marketing to develop and carry out community outreach plans aligned with our mission.
• Partner with clinical staff to ensure referrals transition smoothly into care while prioritizing patient comfort and family support.
• Participate in training and outreach planning, maintaining confidentiality and quality standards.
