Job summary

Gentiva, Heartland Hospice’s parent company, seeks a Hospice Care Consultant to develop referral relationships and champion palliative care solutions. You will educate healthcare professionals, identify growth opportunities, and drive community engagement while supporting patients and families through education and advocacy.

The role emphasizes independent and collaborative work with clinical and operational teams, requiring strong communication and relationship-building skills.

Qualifications

• Bachelor's degree or equivalent experience in a related field.
• Minimum 2 years in healthcare sales, business development, or referral marketing.
• Strong existing relationships with physicians, hospitals, SNFs or home health professionals.

Responsibilities

• Educate, inform, and inspire confidence in hospice services to ensure appropriate referrals.
• Identify new referral opportunities by calling on hospitals, SNFs, home health, clinics, and physician offices.
• Engage in meaningful conversations with healthcare professionals about patient needs and hospice options.
• Plan and execute marketing initiatives to increase awareness and drive referrals.
• Represent Gentiva at community events, associations, and in-services to promote services.
• Serve as a resource for patients and families, highlighting early referral benefits.
