Job summary

Gentiva is seeking a Hospice Care Consultant to champion our mission and cultivate strong referral partnerships with hospitals, home health agencies, SNFs, and physician offices. You will educate healthcare professionals on hospice benefits and patient needs, while driving growth through targeted marketing initiatives.

The role emphasizes relationship-building, strategic outreach, and collaboration with clinical and operations teams to improve patient access and quality of life.

Qualifications

• Bachelor's degree or equivalent experience in business, marketing, communications or related field.
• Minimum 2 years in healthcare sales, business development, or referral marketing.
• Strong existing relationships with physicians, hospitals, SNFs, or home health professionals.
• Experience in hospice, palliative care, or home health is a plus.
• Excellent communication, presentation, and time-management skills.
• Proficiency in Microsoft Office and CRM platforms.

Responsibilities

• Educate, inform, and inspire confidence in our services to ensure the right patients are referred at the right time.
• Identify new referral opportunities by calling on hospitals, home health agencies, SNFs, assisted living communities, clinics, and physician offices.
• Engage in meaningful conversations with healthcare professionals about patient needs and hospice options.
• Plan and execute strategic marketing initiatives to increase awareness and drive referral growth.
• Represent Gentiva at community events, professional associations, and educational in-services.
• Serve as a resource and advocate for patients and families regarding hospice options and referrals.
