Job summary

Enhabit Home Health & Hospice seeks a relationship manager to serve as the main liaison between the company and referral sources including physicians, hospitals and health systems. You will educate partners about services, coordinate post-acute care transitions, and work to improve patient outcomes and satisfaction.

The role requires strong interpersonal and presentation skills, healthcare industry knowledge, and a willingness to work on-site to collaborate with teams and achieve Enhabit

Qualifications

• Bachelor's degree in healthcare administration, marketing, or related field or a graduate of an approved school of nursing, therapy or social work; equivalent experience considered.
• Highly preferred graduate of an approved school of nursing, therapy or social work.
• Minimum two years of experience in healthcare business development, community relations, marketing, or sales.
• Knowledge of federal, state and local laws and regulatory guidelines for Medicare certified home health and hospice operations.
• Valid state driver license and automobile liability insurance required.
• Strong interpersonal, communication and presentation skills; clinical licensure if applicable.

Responsibilities

• Serve as the primary relationship manager between the company and referral sources such as physicians, hospitals and health systems.
• Support referral growth by educating sources about services and program requirements to improve patient outcomes and transitions.
• Promote satisfaction of patients, families and referral sources through effective communication and service delivery.
