Job summary

Enhabit Home Health & Hospice is seeking a relationship-driven professional to serve as a primary liaison between Enhabit and referral sources such as physicians, hospitals, and health systems. The role focuses on growing referrals by educating partners about Enhabit services and program requirements to improve post-acute care transitions and patient outcomes.

Qualifications include a bachelor’s degree in healthcare administration, marketing, or related fields, with healthcare business

Qualifications

• Bachelor's degree in healthcare administration, marketing, nursing, therapy or social work, or equivalent
• Two years of healthcare business development, community relations, marketing, or sales experience
• Understanding of Medicare home health and hospice regulations
• Valid state driver's license and automobile liability insurance
• Excellent interpersonal and presentation skills, both oral and written

Responsibilities

• Serve as the primary liaison between Enhabit and referral sources to grow referrals
• Educate referral sources on services and program requirements to improve post-acute care transitions
• Support care transition and patient/referral source satisfaction through education and relationship building
• Maintain relationships with physicians, hospitals, and senior living communities to expand referral networks
