Job summary

Enhabit Home Health & Hospice seeks a dedicated relationship manager to serve as the primary link between the company and referral sources, including physicians, hospitals, and health systems. You will educate partners about Enhabit services and program requirements to drive referral growth and smooth post-acute transitions.

In this role, you will navigate regulatory guidelines, promote high-quality patient care, and build trusted relationships with care teams.

Qualifications

• Bachelor's degree or equivalent in healthcare/related field or nursing/therapy/social work program.
• Minimum two years in healthcare business development, community relations, marketing, or sales.
• Knowledge of Medicare home health and hospice regulations.
• Valid state driver license and automobile liability insurance.
• Strong interpersonal, communication and presentation skills.
• Clinical license if clinical role is required.
• Preferred experience with home health/hospice and established referral relationships.

Responsibilities

• Act as the primary relationship manager between the company and referral sources (physicians, hospitals, health systems, and other organizations).
• Support referral growth by educating partners about services and program requirements to improve patient outcomes and satisfaction.
• Navigate post-acute care transitions to promote quality care and smooth care coordination.
