Job summary

Eternal Faith Hospice seeks a Patient Care Advocate to develop hospice referral networks in the Athens/Bogart area. You will promote programs, foster partnerships with physicians, facilities, discharge planners, and community agencies, and drive growth while upholding quality end-of-life care standards.

You will meet patients and families to discuss home care, coordinate with Intake and care teams, and monitor referral activity, markets changes, and service delivery to ensure timely,

Qualifications

• Bachelor’s degree in marketing or related field or equivalent professional experience.
• Minimum of two (2) years of experience in hospice sales or related industry with a proven growth record.
• Proficiency in Microsoft programs for communications, presentations, database tracking/reporting, and spreadsheets; CRM experience preferred.

Responsibilities

• Develop business partnerships with physicians, facilities, discharge planners, and community agencies.
• Grow referrals and promote hospice programs and services.
• Maintain professional relationships with providers and referral sources; communicate new products and services.
• Prepare monthly tracking reports on referral sources and market changes.
• Communicate with attending physicians and care teams to coordinate patient care.
