Job summary

Harmony Hospice in the United States seeks a Hospice Liaison to build relationships with physicians, hospitals, SNFs, and other referrals sources, educating professionals about hospice services and guiding families to compassionate care.

You will document outreach, support transitions with care coordination teams, and ensure messaging reflects dignity and quality care. A bachelor’s in Marketing or related field plus 3 years healthcare marketing is preferred.

Qualifications

• Bachelor’s degree in Marketing, Business Administration, or related field preferred.
• Minimum 3 years of healthcare marketing experience, hospice experience strongly preferred.
• Strong communication, relationship-building, and public relations skills.
• Ability to work independently while maintaining a compassionate, patient-first approach.
• Demonstrated organizational skills and ability to meet outreach goals.
• Reliable transportation, valid AZ driver’s license, and proof of insurance.

Responsibilities

• Develop and maintain relationships with physicians, hospitals, skilled nursing facilities, and referral sources.
• Educate healthcare professionals and community groups about hospice services and benefits.
• Serve as a knowledgeable, compassionate resource for patients and families during referral and admission processes.
• Document outreach activities and maintain accurate records of interactions.
• Ensure messaging reflects dignity, comfort, and quality care; support transitions with care coordination teams.
