Unity Hospice WI seeks a Primary Registered Nurse for Meng Residence in De Pere, a 12-bed inpatient hospice facility. The RN collaborates with a team to complete skilled nursing assessments, develop and update care plans, and support interdisciplinary care across hospital-like and home settings.

This full-time role offers a 36-hour week with 12-hour shifts from 7:00 a.m. to 7:30 p.m., plus a fixed Community rotation and occasional on-call and holiday coverage.

Competenze

• Active Wisconsin RN license required.
• 2+ years RN experience preferred.
• Strong communication and teamwork abilities.
• Ability to perform skilled nursing assessments and care planning.

Mansioni

• Provide clinical and case management support for end-of-life care.
• Complete initial, comprehensive and updated patient/family assessments.
• Develop, implement and review care plans; update as needs change.
• Participate in interdisciplinary team meetings and communicate findings.
• Administer and titrate medications per orders; manage medications.
• Delegate nursing duties to LPN and supervise CNA.
