Job summary

Unity Hospice WI is seeking a Registered Nurse to provide compassionate, high-quality care across all Unity service areas in northeast Wisconsin. The role includes admissions, hospital, or community settings with a 0.9 FTE schedule and rotating weekends.

You will work with interdisciplinary teams to assess needs and update care plans, delivering education and crisis support as required. The position requires an Associate degree in Nursing, WI RN licensure, and a valid driver's license.

Qualifications

• Associate degree in Nursing required.
• Bachelor's degree preferred.
• RN license in the state of practice required.

Responsibilities

• Provide skilled nursing assessment and appropriate intervention.
• Update the plan of care and review with patient and family.
• Respond to urgent patient/family needs in collaboration with triage staff.
• Initiate crisis care as needed.
• Record and communicate patient information to the care team.
• Educate patients and families on disease progression and symptom management.
• Titrate medications according to orders.
• Coordinate admissions and patient transportation as needed.
