Job summary

Unity Hospice in Sturgeon Bay, WI is seeking a Primary Registered Nurse (RN) to lead case management for patients in home and facility settings, partnering with float nurses and an interdisciplinary team to deliver high-quality, patient-centered care.

The role includes four 9-hour shifts weekly (7:00 a.m.–4:00 p.m. or 8:00 a.m.–5:00 p.m.), weekend coverage every third weekend (currently every eighth with 12-hour shifts), plus 10–15 on-call shifts and 2–3 holidays per year.

Qualifications

• Current unrestricted Registered Nurse licensure from the State of Wisconsin
• Minimum of 2 years RN experience required, equivalent nursing experience may be considered
• Valid Wisconsin Driver’s License, reliable transportation and proof of current automobile insurance coverage

Primary Registered Nurse (RN) - Sturgeon Bay Territory

Schedule: Four 9-hour shifts per week, with hours scheduled either 7:00 a.m.–4:00 p.m. or 8:00 a.m.–5:00 p.m., depending on POD needs. Weekend coverage will be required at least every third weekend, but currently scheduled every eighth weekend with 12-hour shifts. The position will also require approximately 10–15 on-call shifts per year, and 2–3 holidays per year.

As a Primary Registered Nurse, you will serve as the lead case manager for patients in both home and facility settings. You’ll partner closely with float nurses and an interdisciplinary team to deliver consistent, high-quality, patient-centered care.

This role is ideal for a nurse who values autonomy, teamwork, and meaningful patient relationships. When caseloads fluctuate, you’ll step in to support your team, ensuring every patient receives seamless care.

Territory: Sturgeon Bay area (with occasional travel outside the territory)

***During periods when your caseload is lighter, you will be expected to support your team by assisting with patient visits and transitions, serving as a Float Registered Nurse as needed. This collaborative approach ensures that all patients receive the highest level of care, regardless of fluctuating caseloads. ***
