Job summary

Unity Hospice is seeking a Primary Registered Nurse (RN) to lead case management for patients in home and facility settings in the Green Bay territory, with occasional travel. You will collaborate with float nurses and an interdisciplinary team to ensure high-quality, patient-centered care. Schedule: four 9-hour shifts, 7 a.m.–4 p.m.

Qualifications

• Current unrestricted Registered Nurse licensure from Wisconsin.
• Minimum of 2 years RN experience required, equivalent nursing experience may be considered.
• Valid Wisconsin Driver’s License and reliable transportation.

Responsibilities

• Provide compassionate hospice care and case management.
• Assess patient and family needs; adjust care plans accordingly.
• Respond to urgent situations and initiate crisis care when needed.
• Collaborate with physicians and interdisciplinary team members.
• Educate and support families through end-of-life journey.
• Administer medications and document care in the EMR.
• Delegate and guide LPNs and CNAs.
• Manage daily priorities and assist with admissions.
