Responsibilities

• Grow hospice admissions within a defined Book of Business by prospecting and building physician relationships.
• Develop and deepen connections with physicians, advanced practice providers, practice managers, and referral coordinators.
• Communicate disease state information on how hospice fits into the patient journey.
