Job summary

Gentiva Hospice is seeking a dedicated Patient Care Manager to oversee clinical operations and ensure the delivery of compliant, high-quality hospice services. You will lead patient care, triage referrals, and coordinate care plans while mentoring staff to uphold regulatory standards across a multi-site team.

The role emphasizes strong leadership, clinical documentation, and collaboration with executive leadership to drive performance improvements in patient outcomes and efficiency.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years of direct patient care experience, including 2 years in a hospice or home-based care.
• Knowledge of hospice regulatory standards (Medicare/Medicaid, JCAHO/ACHC).
• Ability to lead a diverse care team and coach staff.
• Strong interpersonal, organizational, and leadership skills.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling and prioritize based on clinical judgment.
• Triage referrals and medical concerns promptly and compassionately.
• Lead IDG meetings, present patient updates, and document minutes.
• Mentor, train, and provide clinical support to associates.
• Ensure compliance with federal, state, and local regulations and company policies.
• Support performance improvement and operational efficiency at the location.
