Job summary

Allegiance Healthcare South Central Hospice is seeking an RN to coordinate patient care according to the Plan of Care. The role involves supervising aides, collaborating with the care team, and maintaining detailed clinical documentation.

Requirements include active Louisiana RN licensure and a minimum of two years' RN experience, hospice experience preferred. Availability for on-call rotation and full- or part-time shifts is expected.

Qualifications

• Licensed registered nurse currently licensed in Louisiana
• Minimum two years full-time RN experience, hospice preferred
• Ability to coordinate and supervise care across the IDT

Responsibilities

• Coordinate POC implementation for each patient
• Document problems, goals, interventions and responses
• Collaborate with attending physician and IDT
• Instruct patient/family in self-care techniques
• Supervise hospice aides and delegate tasks
• Submit clinical notes within one week of visits
• Ensure written care plans for aides are clear and complete
• Conduct supervisory visits every 14 days and annual reviews
• Participate in on-call rotation as needed
