Job summary

Advance Hospice Care is seeking a dedicated Patient Care Manager, RN to oversee clinical operations, coordinate care, and mentor staff within our hospice team. Reporting to the Executive Director, you will ensure delivery of high-quality, compliant hospice services, lead IDG meetings, manage visits, and drive performance improvements while supporting compassionate patient and family experiences.

Qualifications

• RN licensed in the state of employment.
• Graduate of an accredited nursing program.
• At least 3 years direct patient care, including 2 years in hospice/home health.
• Knowledge of hospice principles and regulatory standards (Medicare/Medicaid/JCAHO/ACHC).
• Compassion for terminally ill patients and families.
• Ability to lead a diverse care team with emotional and clinical adaptability.
• Strong interpersonal, organizational, and leadership skills.
• Proficiency in clinical documentation and technology systems.

Responsibilities

• Oversee and direct patient care to ensure high-quality hospice delivery.
• Manage visit scheduling and clinical decision-making.
• Triage referrals and medical concerns promptly and compassionately.
• Lead IDG meetings with patient updates and care plan coordination.
• Mentor and support associates on performance and compliance.
• Ensure care delivery meets regulatory standards and company policies.
