Job summary

Houston Hospice is seeking an experienced Patient Care Manager (RN) to lead an interdisciplinary team delivering compassionate hospice care. The role focuses on developing others, strengthening clinical operations, and ensuring patients and families receive dignity, comfort, and support.

The ideal candidate has strong leadership, regulatory knowledge, and a dedication to quality outcomes. The position requires current Texas RN licensure and a BSN is preferred, with at least five years of

Qualifications

• Current, unrestricted Texas Registered Nurse (RN) license.
• Graduate of an accredited nursing program; BSN preferred.

Responsibilities

• Lead and develop interdisciplinary team members through coaching and mentorship.
• Champion quality patient care by overseeing admissions, plans of care, and service delivery.
• Drive clinical excellence by monitoring documentation, care coordination, and regulatory compliance.
• Facilitate team collaboration through IDT meetings and effective communication with patients, families, and partners.
• Manage daily operations including staffing, assignments, productivity, and budget considerations.
• Support continuous improvement by identifying workflow enhancements and patient/family experience improvements.
• Provide hands-on leadership and serve as a clinical resource during complex patient situations.
