Job summary

Advance Hospice Care seeks a Patient Care Manager to supervise clinical operations and ensure compliant, high-quality hospice services. You will coordinate care, manage scheduling, and mentor staff to uphold standards while navigating regulatory requirements.

The role demands RN licensure, a nursing degree, and several years in direct patient care, including hospice. Strong leadership, documentation skills, and EMR proficiency are essential; BSN is preferred.

Qualifications

• RN license in the state of employment
• Graduate of an accredited nursing program
• Minimum of 3 years direct patient care, including 2 years in hospice or home-based care
• Knowledge of hospice principles, industry standards, and regulatory compliance (Medicare, Medicaid, JCAHO, ACHC)
• Compassion for terminally ill patients and families
• Ability to lead a diverse care team and adapt to emotional/clinical needs
• Strong interpersonal, organizational, and leadership skills
• Competency in clinical documentation and technology systems

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care
• Manage visit scheduling and prioritization based on clinical judgment
• Triage incoming referrals and medical concerns promptly and compassionately
• Lead or facilitate Interdisciplinary Group (IDG) meetings and document minutes
• Mentor and train associates, supporting development and compliance
• Ensure care delivery meets federal, state, local standards and company policies
• Support performance improvement initiatives and operational efficiencies at your location
