Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years of direct patient care experience, hospice or home health preferred.

Responsibilities

• Oversee and direct patient care to ensure high-quality hospice services.
• Lead care coordination and IDG meetings; document minutes.
• Mentor and train staff; ensure regulatory compliance.
