Gentiva Hospice is seeking a Patient Care Manager to lead clinical operations and ensure exemplary, compliant hospice care. Reporting to the Executive Director, you will supervise patient care activities, coordinate staff, and drive continuous improvement in a fast-paced home-based setting.

You will oversee scheduling, triage referrals, and lead interdisciplinary group meetings while mentoring team members and ensuring adherence to Medicare, Medicaid, and JCAHO/ACHC standards.

Qualificações

• RN license in Tennessee required.
• Min. 3 years direct patient care, incl. 2 years in hospice/home health.
• Demonstrated ability to lead diverse care teams and ensure regulatory compliance.
• Knowledge of Medicare/Medicaid, JCAHO/ACHC standards.
• Strong interpersonal and organizational skills.

Responsabilidades

• Oversee and direct patient care to ensure high-quality hospice services.
• Manage visit scheduling and prioritization based on clinical judgment.
• Triage incoming referrals and medical concerns promptly and compassionately.
• Lead IDG meetings, present updates, coordinate care plans, document minutes.
• Mentor and support associates in development, performance, and compliance.
• Ensure care delivery complies with federal/state/local regulations and company policies.
• Support performance improvement initiatives and operational efficiencies at the location.
