Advance Hospice Care in Missouri seeks a Patient Care Manager to lead clinical excellence and ensure high-quality hospice services. Reporting to the Executive Director, you will supervise patient care, coordinate visits, and drive staff development while maintaining regulatory compliance.

You will mentor staff, lead IDG meetings, and ensure care delivery aligns with Medicare/Medicaid and ACHC standards. This is a frontline leadership role with a strong emphasis on compassionate, compliant

Formación

• RN license in state of employment is required.
• Graduate of an accredited nursing program.
• Minimum of 3 years direct patient care, including 2 years in hospice/home-based care.
• Demonstrated knowledge of hospice principles, regulatory standards (Medicare/Medicaid/JCAHO/ACHC).
• Ability to lead a diverse care team and adapt to patient needs.
• Strong interpersonal, organizational, and leadership skills.
• Competent in clinical documentation and technology systems.

Responsabilidades

• Oversee and direct patient care to ensure high-quality hospice services.
• Manage visit scheduling and prioritize based on clinical judgment.
• Triage referrals and medical concerns promptly and compassionately.
• Lead or participate in Interdisciplinary Group (IDG) meetings with patient updates and care plans.
• Mentor and support associates to ensure performance and compliance.
• Ensure care delivery meets regulatory standards and company policies.
• Support quality initiatives and operational efficiencies at the location.
