Job summary

JourneyCare Hospice is seeking a Hospice After Hours Patient Care Manager to join our team. New competitive salaries, immediate opening available, generous time off packages, 401K + match and so much more!

Hours: Monday-Friday 3PM-11PM with on-call 11PM-8AM and one weekend shift every 12th weekend. Territory includes Barrington, Deerfield, Crystal Lake, or Palos Heights with remote options after training and in-office 3 days per week.

Qualifications

• Associates degree preferred or equivalent healthcare experience.
• Minimum of two years’ hospice experience required.
• Experience dealing with Medicare/Medicaid and other payers preferred.
• Customer service oriented with ability to put clients at ease and address needs.

Responsibilities

• Coordinate information with clients, family members, physicians, referral sources, facility staff and agency staff to ensure continuity of care.
• Inform referral sources and clinical staff about response times and follow-up plans; document information in the medical record.
• Input patient information into the EMR and update the level of care as needed.
• Prepare and copy necessary paperwork for patient admission; manage EMR and paper charts per guidelines.
• Verify insurance coverage and collaborate to obtain required information.
• Create morning and evening census reports.

Qualifications:

• Associates degree is preferred and equivalent with successfully completed course work or equivalent experience in medical terminology and/or working in healthcare environments.
• Minimum of two years’ experience working in hospice REQUIRED.
• Experience in dealing with a variety of payers in healthcare, e.g., Medicare, Medicaid, and commercial payors, preferred.
• Customer service focused, including the ability to set the caller at ease and encourage open dialogue about the client’s needs.
• Ability to work with a variety of team members representing other functional areas that directly or indirectly impact client intake/admission.
• Ability to maintain composure in addressing stressful and/or challenging situations.
• Prior use of Electronic Medical Records (EMR), preferred.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.

HOURS

Monday-Friday 3PM-11PM

On-Call Expectations

Monday-Friday 11PM-8AM plus One Shift Every 12th Weekend (either a Saturday or Sunday)

Territory

Barrington, Deerfield, Crystal Lake, or Palos Heights Office Options Available - Position is Remote after Training but Expected in office 3 Days Per/Week and Onsite for Weekend Rotation.

What You’ll Do:

• The Intake Manager will ensure the team is providing initial coordination of information with clients, their family members, physicians, referral sources, facility staff, agency staff, and any others involved in assuring continuity of client care.
• Inform referral sources, clients, family members, and physicians about response time and plans for follow up. Informs clinical office staff of potential admits and accurately documents information in the client’s medical record.
• Input patient information into the EMR and updates EMR with the correct level of care.
• Completes and copies necessary paperwork to ensure patient files are prepared for admission. This includes but is not limited to completing the referral intake form, copying all referral information, and attaching it to the intake for review by clinical office staff, scanning and uploading referral information in a timely manner so information is available to admissions clinicians, Establishes, maintains, and closes patient’s EMR and paper charts in accordance with regulatory and office guidelines. This may include but is not limited to the mailing, faxing, or emailing of required documentation.
• Verification of insurance coverage. Collaborates as needed to procure needed information.
• Creates morning and evening census reports.
