Job summary

JourneyCare Hospice in Chicago, IL is seeking a compassionate Hospice RN to join our team and deliver high-quality patient-centered care.

You will assess physical, emotional, social, and spiritual needs, coordinate with the interdisciplinary team, and support families through the end-of-life process.

Schedule is Monday–Friday 4:30PM–12:30AM with every 4th weekend; we offer generous time off, 401(k) with match, tuition reimbursement, and employee referral programs.

Qualifications

• Graduate from an accredited registered nursing program.
• Current RN license to practice in Illinois.
• Minimum of two years as a professional nurse.
• CPR certification is required.
• Ability to cope with family emotional stress.

Responsibilities

• Obtain data on physical, psychological, social, and spiritual factors affecting patient and family needs and coordinate with the interdisciplinary team.
• Communicate with attending physician and other staff to coordinate optimal care.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate clinical records with care plans and progress notes.

What You’ll Do:

• Obtain data on physical, psychological, social, and spiritual factors that may impact patient and family’s needs and initiate intervention by other members of the interdisciplinary team as appropriate
• Initiate communication with the attending physician, other interdisciplinary staff members and other agencies for the purpose of coordinating optimal care.
• Perform clinical pronouncement of death and provide support to family
• Maintain an accurate clinical record through the timely preparation of care plans and clinical and progress notes

Salary: $80,000 - $97,000 Annually - Offer Based on Years of Experience
