Job summary

JourneyCare Hospice is seeking a Hospice After Hours Patient Care Manager to join our team. New competitive salaries, immediate opening available, generous time off packages, 401K + match and so much more!

Hours: Monday-Friday 3PM-11PM with on-call 11PM-8AM and one weekend shift every 12th weekend. Territory includes Barrington, Deerfield, Crystal Lake, or Palos Heights with remote options after training and in-office 3 days per week.

Qualifications

Associates degree preferred or equivalent healthcare experience.
Minimum of two years’ hospice experience required.
Experience dealing with Medicare/Medicaid and other payers preferred.
Customer service oriented with ability to put clients at ease and address needs.

Responsibilities

Coordinate information with clients, family members, physicians, referral sources, facility staff and agency staff to ensure continuity of care.
Inform referral sources and clinical staff about response times and follow-up plans; document information in the medical record.
Input patient information into the EMR and update the level of care as needed.
Prepare and copy necessary paperwork for patient admission; manage EMR and paper charts per guidelines.
Verify insurance coverage and collaborate to obtain required information.
Create morning and evening census reports.
