Job summary

JourneyCare Hospice in Chicago, IL is seeking a skilled Registered Nurse to join our interdisciplinary team providing compassionate end-of-life care.

The role requires graduation from an accredited nursing program, active RN license, CPR certification, and a minimum of two years of professional nursing experience. Schedule: Mon-Fri 4:30PM-12:30AM with every 4th weekend; Territory: North Chicago area. You will coordinate care with physicians and staff, document accurately, and support families.

Qualifications

• Graduate from an accredited registered nursing program.
• Current license to practice in Illinois as a Registered Nurse.
• Minimum of two years of varied work experience as a professional nurse.
• Demonstrated skill in nursing practices and clinical judgments.
• Ability to cope with family emotional stress and tolerance of individual lifestyles.
• Must possess CPR Certification.
• Valid driver’s license and proof of insurance.
• Schedule: Mon-Fri 4:30PM-12:30AM with every 4th weekend. territorial coverage: North Chicago, IL.

Responsibilities

• Obtain data on physical, psychological, social, and spiritual factors affecting patient and family needs and initiate interdisciplinary interventions.
• Coordinate care by communicating with the attending physician and other staff for optimal treatment.
• Perform clinical pronouncement of death and provide family support.
• Maintain accurate care plans and progress notes through timely documentation.

What You’ll Do:

• Obtain data on physical, psychological, social, and spiritual factors that may impact patient and family’s needs and initiate intervention by other members of the interdisciplinary team as appropriate
• Initiate communication with the attending physician, other interdisciplinary staff members and other agencies for the purpose of coordinating optimal care.
• Perform clinical pronouncement of death and provide support to family
• Maintain an accurate clinical record through the timely preparation of care plans and clinical and progress notes

Salary: $80,000 - $97,000 Annually - Offer Based on Years of Experience
