Job summary

Chapters Health System is seeking a Clinical Team Leader (CTL) of Patient Access to oversee the daily intake process and clinical coordination for hospice patients and families. You will support staff delivering care per the plan of care and ensure timely processing of referrals.

The role requires an RN license, hospice experience, BLS certification, and strong communication skills to lead teams and manage complex patient needs in a dynamic care environment.

Qualifications

• Current RN license in the state in which you work.
• Minimum of three (3) years’ nursing experience, with one (1) year hospice experience.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system.
• Excellent critical decision making and problem-solving skills.
• Ability to convey a high level of urgency.

Responsibilities

• Acts as a clinical resource for clinical and office staff (i.e., Care Access Coordinators, Care Access RNs, etc.).
• Oversees intake staff to ensure accurate, efficient, and timely processing of referrals and responses to referral sources.
• Assists scheduling staff with escalations from the Care Navigation Center in collaboration with the Care Access Supervisor and/or Director of Patient Access.
• Maintains relationships with referral sources and internal customers to meet the needs for efficient processing of patient information.
• Recommends alternate approaches to patient care in collaboration with the Admissions Managers and Director of Patient Access, while ensuring that policies and procedures are followed.
• Collaborates with other departments to ensure patient care needs are addressed. Participates in complex patient care conferences as needed.
• Demonstrates respect and understanding for all clinical disciplines required for quality patient care and mentors staff in this process.
• Communicates in a clear and accurate manner. Provides constructive feedback and resolves conflict directly.
