Job summary

Capital Caring Health in Maryland seeks a Clinical Team Leader of Patient Access to oversee the intake process, coordinate clinical support, and ensure timely referrals for hospice care.

The role requires an active RN license, hospice experience, and BLS; you will lead Care Access staff, collaborate with admissions, and promote outstanding patient care. Travel to patient locations is expected.

Qualifications

• Current RN license in the state where you work.
• Minimum of three (3) years’ nursing experience, with one (1) year hospice experience.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system
• Ability to travel off-site to meet business needs.

Responsibilities

• Acts as a clinical resource for Care Access staff.
• Oversees intake staff to ensure accurate, efficient, and timely processing of referrals.
• Assists scheduling staff with escalations from the Care Navigation Center in collaboration with the Care Access Supervisor/Director of Patient Access.
• Maintains relationships with referral sources and internal customers to meet needs for efficient processing of patient information.
• Collaborates with Admissions Managers and Director of Patient Access to ensure policies are followed.
• Mentors staff in clinical processes and communicates clearly, providing feedback.
• Ensures timely attention to patient concerns and delivers outstanding customer care.
