Clinical Team Leader (CTL) of Patient Access
In the Chapters Health Team, the CTL oversees the daily intake process, clinical coordination, and staff support for hospice care as outlined in each patient’s plan of care.
Qualifications
- Current RN license in the state of employment, a single‑state out‑of‑state license, or a multistate/compact license.
- Minimum three (3) years of nursing experience, including at least one (1) year in hospice.
- Active BLS certification from the American Heart Association or RedCross.
- Experience with an EMR/EHR system.
- Physical ability to access any home, including stairs, narrow spaces, heat conditions, and the ability to lift patients or equipment.
- Valid driver’s license with automobile insurance and willingness to travel off‑site as required.
- Ability to manage emotional stress and handle crisis situations calmly.
- Excellent critical decision‑making, problem‑solving, and communication skills.
- Professionalism, confidentiality, and strong adherence to regulatory requirements.
Responsibilities
- A clinical resource for care‑access staff, ensuring timely, accurate referral processing.
- Oversee intake staff, manage scheduling escalations, and maintain relationships with referral sources and internal customers.
- Recommend alternative patient‑care approaches in collaboration with admissions managers and patient‑access leadership.
- Collaborate with medical services, pharmacy, infusion, and continuous care to address patient needs.
- Participate in complex patient‑care conferences as needed.
- Mentor staff, provide constructive feedback, and improve communication skills.
- Address customer concerns and complaints, ensuring outstanding customer care.
- Perform other duties as assigned.

Regulatory Compliance
- Educate and monitor staff on regulatory responsibilities.
- Maintain compliance with Medicare Conditions of Participation and state hospice regulations.
Compensation
Pay range: $85,806.28 – $127,714.07.
