Zusammenfassung

AccentCare is seeking a Hospice Care Consultant for the Indianapolis area. You will build relationships with healthcare providers and facilities, educate referral sources about hospice services, and ensure timely, compassionate patient care.

You will lead outreach, manage a territory, and collaborate with Admissions and Marketing to grow referrals while upholding the highest ethics and patient-centered care.

Qualifikationen

• Bachelor’s degree and/or 5 years of proven experience in home health, healthcare sales, or hospice sales.
• Strong communication, problem-solving and relationship-building skills.
• Ability to work independently and manage a territory effectively.
• Knowledge of hospice regulations and services preferred.

Aufgaben

• Develop and maintain relationships with physicians, hospitals, SNFs, and community organizations through in-person visits and educational conversations.
• Drive hospice sales by nurturing current relationships, identifying new referral opportunities, and increasing program awareness.
• Conduct presentations and in-services to educate partners on hospice benefits and eligibility.
• Collaborate with Admissions and Marketing to create outreach plans aligned with the mission of honoring life and offering hope.
• Partner with clinical staff to ensure referrals transition smoothly into care.
• Participate in training and planning outreach strategies while upholding confidentiality.
