Job summary

AccentCare, Inc. is seeking a Hospice Care Consultant in Indianapolis to build relationships with healthcare providers, facilities and community partners. You will lead hospice sales, educate referral sources about services, and ensure timely, quality patient care.

Qualifications

• Bachelor’s degree or equivalent experience in home health, healthcare sales, or hospice sales.
• Strong communication, problem-solving and relationship-building skills.
• Ability to work independently and manage a territory.
• Knowledge of hospice regulations and services preferred.

Responsibilities

• Develop and maintain trusting relationships with physicians, hospitals, SNFs, and community organizations through in-person visits and educational conversations.
• Drive hospice sales by nurturing current relationships and identifying new referral opportunities.
• Conduct presentations and in-services to educate partners on hospice benefits and eligibility.
• Collaborate with Admissions and Marketing to develop and execute community outreach plans.
• Partner with clinical staff to ensure referrals transition smoothly into care with patient dignity in mind.
• Participate in training, planning outreach strategies, and uphold confidentiality.
