Job summary

Cabrillo Hospice in Riverside County seeks an experienced Hospice Account Executive to manage referral partner relationships and coordinate patient transitions into hospice care. You will educate partners on eligibility and services while collaborating with clinical teams to ensure seamless care.

Based in the Inland Empire, the role focuses on building long-term partnerships with hospitals, SNFs, physician networks, and other healthcare providers to support high-quality, patient-centered care.

Qualifications

• Experience in hospice, home health, or healthcare account management.
• Understanding of hospice philosophy, eligibility, and interdisciplinary care.
• Minimum two years of experience with physicians, hospitals, insurers, or healthcare partners.
• Strong communication and relationship-building skills.
• Excellent organizational skills to manage multiple accounts.
• Ability to work independently and as part of a multidisciplinary team.
• Valid driver's license and reliable transportation.

Responsibilities

• Develop, manage, and maintain relationships with referral partners, including hospitals, physicians, skilled nursing facilities, assisted living communities, and other healthcare providers.
• Serve as primary contact for referral sources with clear, responsive communication.
• Collaborate with discharge planners, case managers, social workers, and clinical teams for timely transitions into hospice care.
• Educate referral sources on hospice eligibility, services, and care coordination processes.
• Work with interdisciplinary hospice team to align patient care goals and improve workflows.
• Represent Cabrillo Hospice at community events and professional gatherings.
