Job summary

Cabrillo Hospice is seeking an experienced Hospice Account Executive in the San Diego area. The role focuses on managing referral partner relationships, care coordination, and collaboration with clinical teams to ensure smooth transitions into hospice care.

The position emphasizes relationship-building and education of referral sources about eligibility and services, while representing Cabrillo Hospice at community events and maintaining strong partnerships across hospitals, SNFs, and physicians.

Qualifications

• Experience in hospice, home health, or healthcare account management strongly preferred.
• Minimum of two years of experience working with physicians, hospitals, insurance providers, or other healthcare partners.

Responsibilities

• Develop, manage, and maintain strong relationships with referral partners including hospitals, physicians, SNFs, and other providers.
• Serve as primary contact for referral sources with consistent communication and responsiveness.
• Collaborate with discharge planners, case managers, social workers, and clinical teams for smooth patient transitions into hospice care.
• Educate referral sources on hospice eligibility, services, and care coordination processes.
• Work with the interdisciplinary hospice team to align patient care goals and improve referral workflows.
• Represent Cabrillo Hospice at community events and professional gatherings.
• Identify opportunities to improve communication, service delivery, and partner satisfaction.
