Job summary

Anchor Health in Riverside and San Bernardino County is seeking a Hospice Clinical Liaison to grow patient census and serve as a positive ambassador for our hospice program within the community. The ideal candidate is relationship-driven, proactive, and excels at building partnerships with physicians, hospitals, and skilled nursing facilities.

LVN/RN/MSW are preferred, with 1–2 years in healthcare marketing/sales and at least 2 years hospice experience.

Qualifications

• LVN, RN, or MSW required.
• 1–2 years of healthcare marketing/sales experience preferred.
• Minimum of 2 years hospice experience highly preferred.
• Familiarity with local referral partners including hospitals, physicians, B&C, ALF, RCFE, SNF.
• Excellent written and verbal communication skills.
• Self-directed with the ability to work independently.

Responsibilities

• Build and maintain relationships with physicians, hospitals, skilled nursing facilities, discharge planners, and community partners.
• Plan and coordinate marketing events, educational sessions, and community luncheons.
• Educate healthcare stakeholders on hospice and palliative care services.
• Support patients, families, and referral partners throughout the referral and admission process.
• Provide hospice informational / goals of care conversations with patients and families.
• Facilitate initial patient consent process.
• Attend weekly meetings to report successes, share competitor insights, and identify growth opportunities.
• Meet or exceed performance expectations set by the Director of Business Development and Executive Director leadership
