Constellation Health Services is seeking a Nurse Practitioner to deliver comprehensive hospice care in Cincinnati. You will assess patients in home settings, develop individualized plans of care and coordinate with an interdisciplinary team under physician supervision.

The role emphasizes symptom management, eligibility assessments, and education of patients and families. A valid NP license, DEA, CPR, and hospice experience are required; CHPN is a plus.

Qualifikationen

• Grad from an accredited School of Nursing and accredited NP program.
• Current DEA license and state nursing licensure.
• CPR certification and valid driver's license as required by state.
• Experience in home care/hospice is required.
• Certification in Hospice and Palliative Nursing (CHPN) preferred; other specialties acceptable.
• Ability to lift and transfer patients, and readiness for walking/standing.

Aufgaben

• Provide medical care, palliate and manage terminal illness under supervision of a collaborating physician.
• Perform history and physical examinations and document findings.
• Develop plan of care, prescribe medications, coordinate hospice services.
• Complete hospice face-to-face visits to assess eligibility.
• Collaborate with hospice medical director on eligibility.
• Coordinate care with interdisciplinary team.
• Ensure symptom control through team collaboration.
• Develop treatment protocols and propose interventions based on literature.
• Consult with physician when plan deviates from standard practice.
• Educate patients, families, and caregivers on preventative, medical, psychological and spiritual issues.
• Maintain accurate clinical notes and documentation.
